The healing properties of koumiss, as well as any remedy from traditional medicine, have undoubtedly been known for a long time.

In Kazakhstan, Kyrgyzstan, Uzbekistan, as well as Bashkortostan, Tatarstan, Kalmykia and other republics and regions of Russia, koumiss is not only a favorite drink, but also a remedy. In many resorts of the country, koumiss has long been used to treat patients with tuberculosis and other diseases.

The development of koumiss production and koumiss treatment is associated with the names of N.V. Postnikov, A.N. Rubel, P.Yu. Berlin, A.V. Segrista, M.N. Karnaukhova, Y.A. Saygin, M.G. Kuramshina, L.M. Horowitz-Vlasova, N.A. Kramova , Z.Sh.Zagidullina, G.N.Teregulov, S.V.Bazanova and many others.

The scientific substantiation of koumiss treatment is associated with the name of Dr. N.V. Postnikov. He was the first, on the basis of a deep and comprehensive study of koumiss treatment in the sanatorium organized by him, in three words - “nourishes, strengthens, renews” - expressed the essence of the action of koumiss on the body. The closest friend and scientist of N.V. Postnikov, the English scientist George Carrick, describes this cradle of Russian sanatorium construction as follows: “In 1958, Dr. N.V. an institution 6 versts from Samara with the aim of treating consumption and other debilitating diseases with fermenting mare's milk. Further, in his famous book “On koumiss”, D. Carrick writes that “in a few years, koumiss gained fame not only throughout Russia, but also in neighboring countries, as the most effective remedy against consumption.”

First of all, it is necessary to mention the original research of Professor A.N. He carefully developed a sanatorium regimen and indications for koumiss treatment. In general, A.N. Rubel considered koumiss treatment as an active, strengthening, training method of treating patients with predominantly early forms of tuberculosis and required the improvement of sanatoriums. As you know, A.P. Chekhov was treated with Bashkir koumiss in the Andreevsky sanatorium.

Professor P.Yu.Berlin, who worked for many years in the Shafranovsky resort together with Professor L.I.Model, is rightfully considered the father of modern koumiss treatment. Together with their students and followers, they managed to prove that mare's milk and koumiss contain 4-6 times more vital vitamins than cow's.

Koumiss favorably affects the central nervous system, hematopoiesis, improves appetite, food absorption, normalizes the secretory and motor functions of the digestive organs and has an irritating effect on the respiratory center as a whole, causing a reaction in it that leads to the resorption of inflammatory foci (in particular, tuberculous infiltrates) .

A significant contribution to this doctrine was made by the works of scientists from Bashkortostan. B.V. Suleymanov proved by chemical and biological methods that vitamin C is contained in sufficient quantities in winter koumiss. In other words, the horse's body is able to endogenously obtain vitamin C. Of exceptional interest are the antibiotic properties of koumiss yeast and koumiss sticks.

Back in 1940 V.M.Markov-Osorgin(Bashkir Medical Institute) proved that after the combination of a daily culture of koumiss sticks with cultures of dysentery, typhoid and paratyphoid, a rapid death of infectious microbes is observed. Later, professors M.G. Zhilin (Orenburg), E.P. Soloviev, A.A. Shuster (Ufa), M.G. Kuramshina (Alma-Ata) proved that koumiss yeast, which forms lactose, causes the death of tubercle bacilli . A well-known specialist in the treatment of koumiss in Bashkiria, V.K. In particular, the authors pointed to the beneficial effect of koumiss on the cardiovascular system with the correct dosage in tuberculosis patients. A step forward was the use of koumiss for urogenital tuberculosis. Professor L.P.Kraizelburd, which, on the basis of rigorous testing, boldly broke the empirically established idea of ​​​​contraindications for the use of koumiss in kidney tuberculosis.

The use of koumiss in the sanatoriums of Bashkortostan is by no means limited to tuberculosis. Professor G.N. Teregulov has long proved the beneficial effect of koumiss in chronic bronchitis, pneumoclerosis, pneumoconiosis. With great benefit, koumiss is used in chronic gastrointestinal diseases.

According to Professor Z.Sh.Zagidullin and the well-known koumiss therapist ZV Askarova, the treatment of chronic gastritis with secretory insufficiency with koumiss is a very effective remedy. The observations of Professor S.V. Bazanova proved that the treatment of functional disorders of the intestines (stomach) and gallbladder with koumiss can be successfully used in an extra-resort setting (in a hospital or at home). For the first time in the history of koumiss treatment, Professor S.V. Mikhailovsky successfully used koumiss in the treatment of the ear, throat, nose in the form of instillations in the Yumatovo sanatorium.

In the problem of longevity, koumiss plays an important role. The most important measure of protection against premature aging should be considered lipotropic substances, which are rich in koumiss. It is no coincidence that about 100,000 people over the age of 70 live in Bashkortostan, and, moreover, mainly in areas where koumiss is prepared.

Kumis is a product of alcoholic and lactic acid fermentation of mare's milk under certain conditions of temperature and aeration. Comparison of the composition of mare's milk and koumiss allows us to assert that the matter is not limited to one fermentation, deeper physico-chemical processes take place in koumiss. According to the generally accepted opinion of P.Yu.Berlin, koumiss is a biological substance that changes all the time due to the vital activity of lactic acid bacteria and yeast.

Koumiss contains easily digestible proteins and fats, milk sugar, lactic acid, carbon dioxide, alcohol, a large amount of vitamins, enzymes, and minerals. Essential Nutrients components of koumiss (proteins, fats, sugar) are absorbed almost completely (up to 95%). Kumis helps to increase the digestibility of fats and proteins contained in food, especially in meat.

The many-sided beneficial effect of koumiss is explained by its biostimulant properties. It has been established that koumiss has a tonic effect on the nervous system, enhances redox processes, regulates acid-base balance, increases metabolism, stimulates the activity of the cardiovascular system and the respiratory center, secretory, motor function of the gastrointestinal tract, reduces the processes of putrefaction and fermentation in the intestines, intoxication, revitalizes hematopoiesis - increases the number of red blood cells and hemoglobin, stimulates lymph formation, increases the protective function of the liver, increasing glycogen stores in it, etc.

Koumiss successfully combines a complex of many valuable nutrients and antibiotic substances. At present, science has numerous data from experimental and clinical studies that confirm the healing properties of koumiss.

The great successes of antibacterial therapy of tuberculosis and the achievements of pulmonary surgery did not reduce interest in koumiss treatment. The popularity of koumiss and koumiss cure is growing. Numerous scientific studies and an increase in the number of koumiss treatment sanatoriums testify to this.

It is well known that the progressive course of the tuberculous process is accompanied not only by local, destructive changes, but also by a sharp decrease in the immunobiological and reactive capabilities of the organism. Observations of phthisiatricians proved that koumiss treatment leads to an increase in the function of the body's natural defense mechanisms, creates a more stable, easily coordinating balance of organ functions.

The conducted literature review and our observations showed that during koumiss treatment, the drug resistance of microbacteria develops relatively more slowly, and the patients themselves tolerate antibacterial drugs better. Studies of surgeons have shown that after koumiss treatment, patients tolerate surgery more easily and the postoperative period proceeds with fewer complications, as a result of which the effectiveness of surgical treatment increases. Kumis has antibacterial properties.

Our special observations of patients who received chemotherapy in combination with koumiss showed that the frequency of side effects in chemotherapy in combination with koumiss is significantly lower than in medical institutions without koumiss treatment. As is known from the literature, the frequency of side effects ranges from 15.7% (D.D. Yablokov) to 25.0% (N.A. Shmelev), and complete intolerance to drugs reaches 4.8% (N.A. Shmelev). In koumiss sanatoriums of Bashkortostan, the frequency of side effects ranged from 0.7 to 6.0%, respectively, and complete intolerance to the drugs was noted only in 0.7% of patients.

Of interest is a group of patients of 56 people, the observation of which was carried out at the Shafranovo resort. According to dispensaries, 10 of them did not tolerate streptomycin, 29 - PAS, 14 - all drugs GINK, 3 - ethionamide. After taking koumiss for 3-4 weeks, these patients were prescribed, against the background of koumiss treatment, exactly those drugs, the tolerance of which was indicated. 8 patients out of 10 who were prescribed streptomycin, 20 out of 29 who were prescribed PAS, tolerated these drugs well. All 14 patients who did not tolerate GINK and 3 who did not tolerate ethionamide also tolerated them well.

These observations gave us the opportunity to make sure that koumiss treatment significantly improves the tolerability of chemotherapy drugs. Our opinion was also confirmed in the report of N.S. Ponomareva (1969).

Such an effect of koumiss is confirmed by certain aspects of the action of koumiss on the body:

1. Koumiss is a multivitamin drink, and vitamins, especially group B, contribute to better tolerance of drugs.

2. As A.V. Segrist also found out, koumiss, by normalizing disturbed metabolism, increases the body's defenses not only against tuberculosis, but also other harmful agents.

3. Koumiss, being well absorbed by itself, provides better digestibility of proteins present in food products (L.M. Model). And enhanced protein nutrition facilitates the tolerance of chemotherapy drugs.

After the analysis of the results of treatment of tuberculosis patients with the use of koumiss in the sanatoriums of Bashkortostan and without koumiss in the sanatoriums of the Chelyabinsk and Sverdlovsk regions, we became aware that koumiss treatment significantly increases the effectiveness of the treatment of various manifestations of the tuberculosis process, which prompted us to pay special attention to the issues of proper organization of koumiss production and ensuring patients with high-quality koumiss throughout the year.

Koumiss is most widely used in the treatment of diseases of the digestive system, it has a beneficial effect on the functional state of these organs. At the same time, in the literature, the normalizing effect of koumiss was noted both with increased and decreased acidity of gastric juice, which was the basis for its use in peptic ulcer disease.

Even in the last century, D. Carrick (1885) pointed out the possibility of treating peptic ulcer with koumiss. The materials of past years are consistent with the latest reports in the literature on the successful use of fermented milk products (A.M. Skorodumova, 1961; V.S. Vazlina, A.V. Kompaneytseva et al., 1963; P.V. Smoligovets, 1963; V.P. Sokolovsky, 1968; L.M. Slipchenko, 1970), as well as the use of koumiss for the treatment of peptic ulcer of the stomach and duodenum (P.Yu. Berlin, 1960; M.N. Karnaukhov, 1964; V.S.Staroverova, 1964; R.Ya.Khasanov, 1969). Meanwhile, taking into account the rather widespread use of natural koumiss in Bashkortostan and taking into account the existing literature data on the normalizing effect of koumiss treatment on the digestive organs, we considered it appropriate to include fresh natural koumiss in the treatment of patients with gastric and duodenal ulcers in the conditions of the climatic koumiss treatment sanatorium "Yumatovo" Republic of Bashkortostan. The aim of our work was to study the therapeutic value of koumiss in the general complex of measures for the treatment of patients with peptic ulcer of the stomach and duodenum, to clarify the method of its appointment, dosage and treatment efficacy.

To accomplish this task, we conducted a clinical study, monitored the dynamics of the secretory-evacuation and other functions of the stomach in patients with gastric ulcer and duodenal ulcer.

Kumis, being a valuable therapeutic and dietary remedy, has a diverse and versatile effect on the body.

Under our supervision there were 160 patients, of which 130 were in the main and 30 in the control groups, the last in the complex treatment koumiss was replaced by mineral water "Borjomi" in other similar cases, out of 130 patients of the main group with gastric ulcer there were 28 people, duodenal ulcer - 102 people, men - 118 (90.8%) and women - 12 (9.2%), most aged 20 to 50years and with a long course of the disease. All patients upon admission to the sanatorium had complaints and expressed objective clinical symptoms of the disease.

Apart from general clinical and physical research methods, clinical and laboratory studies of all patients before and after treatment were carried out. The content of total protein and protein fractions, cholesterol in blood serum was studied, some indicators of the function of the adrenal cortex and radiological data were studied.

The study of the results of complex therapy with the use of koumiss in patients with peptic ulcer who underwent treatment was determined on the basis of the criteria for the effectiveness of sanatorium treatment developed by the Central Institute of Balneology and Physiotherapy of the Ministry of Health of the USSR (M.M. Mazur, 1965; Yu.E. Danilov, 1973).

As a result of the complex treatment in combination with koumiss in patients with peptic ulcer, along with an improvement in the general condition (appearance of vivacity, improved well-being, normalization of sleep), koumiss treatment led to a significant decrease and disappearance of dyspeptic phenomena: belching stopped in 23 out of 26 patients; heartburn disappeared in 59 out of 69; nausea resolved in 48 of 51;vomiting in 16 of 16 patients. Pain syndrome, identified at the beginning of treatment in 130 patients, disappeared in 112 and decreased in 17 patients, and in most cases, a decrease dyspeptic phenomena and pain syndrome occurred in the first week of treatment. There was also an improvement in appetite and normalization of bowel function. A direct radiological sign of peptic ulcer - a "niche", established before treatment in 25 patients, after treatment was found in 5. Cicatricial deformity of the duodenal bulb was established in 79 patients, after treatment, the majority showed a decrease in swelling of the folds of the gastric mucosa and duodenum, normalization of tone and evacuation function of the stomach.

The results of complex treatment in combination with koumiss for patients with peptic ulcer of the stomach and duodenum, carried out on the basis of the criteria for the effectiveness of sanatorium treatment at resorts for patients with peptic ulcer of the stomach and duodenum, amounted to: out of 130 patients with peptic ulcer of the stomach and duodenum of the main group who underwent treatment , were discharged from the sanatorium with an assessment: significant improvement - 13%, improvement - 86.1% and no change - 0.9% of patients.

The long-term results were studied according to the data of the questionnaires according to the form developed by us and during the repeated course of treatment in the Yumatovo sanatorium. They were traced in 100 patients, of which stomach ulcer was in 8 and duodenal ulcer - in 92 patients. Analysis of the data obtained in the study of long-term results of complex treatment in combination with koumiss showed that in most of the examined individuals, the favorable immediate effect of therapy was more stable. All patients responded well to the course of koumiss treatment and wished to repeat the course of treatment in a sanatorium. Persistent improvement after the last course of treatment was noted in 39% of patients who had no exacerbation of the disease for two years and were able to work, 44% of patients had good results during the year and only a year later noted the appearance of pain in the epigastric region, without dyspeptic symptoms. phenomena. In 17% of patients, exacerbation of peptic ulcer occurred after 6 months, which was associated with a violation of the diet and neuropsychic moments and required inpatient treatment by the end of the first year after sanatorium treatment. Taking into account the literature data (M.Y. Pevzner, 1947; V.S. Azarova, 1950; O.L. Gordon; 1957), the long-term results of the complex therapy we used in combination with the intake of koumiss should be regarded as quite satisfactory. The absence of recurrence of the disease within two years in 39% and within one year in 44% of patients indicates a positive effect of the proposed treatment complex.

The study of the control group of patients with peptic ulcer of the stomach and duodenum for a comparative assessment of the effectiveness of treatment revealed the high efficiency of complex sanatorium treatment with the use of koumiss.

Successful clinical observations allowed us to include among the indications for treatment in the Yumatovo sanatorium patients with peptic ulcer of the stomach and duodenum and open a specialized department for the treatment of diseases of the digestive system.

Later studies, based on laboratory data and experiments on dogs, showed that koumiss has a strong sap action on the digestive glands. This action of koumiss is mainly due to the presence of alcohol and lactic acid. Koumiss enhances secretion both when taken before meals and during meals, so it is useful for patients with high acidity of gastric contents to drink koumiss before meals. Duodenal digestion also improves due to increased pancreatic secretion (trypsin, amylase and lipase) and increased bile formation.

Koumiss also affects the motor function of the stomach, increasing peristalsis and causing spasms of the colon. Koumiss, due to the high content of lactic acid bacteria, weakens putrefactive processes in the colon.

Thus, koumiss helps to increase the acidity of the stomach in patients with anacid and hypoanacid gastritis, lower acidity at its high numbers (which is much less common), improves the motility of the gastrointestinal tract, reduces putrefactive processes in the intestine - all this helps to improve well-being and general condition sick.

Given the above properties, koumiss is used in the treatment of diseases of the gastrointestinal tract (chronic gastritis with secretory insufficiency, peptic ulcer of the stomach and duodenum, operated stomach, biliary tract); chronic nontuberculous lung diseases; cardiovascular diseases (hypertension, atherosclerosis); anemia and some diseases accompanied by metabolic disorders and vitamin deficiency.

We, under the Ural zonal administration of special sanatoriums of the Ministry of Health of the Russian Federation, on a voluntary basis of the commission for koumiss production and koumiss treatment, created a body for coordinating the issues of koumiss horse breeding, production of koumiss and koumiss treatment, which included livestock specialists, bacteriologists and clinicians familiar with the issues of koumiss production and koumiss treatment. This commission launched active activities to improve the breed of koumiss mares, increase milk yield, and improve the technology of koumiss production.

Even in sanatoriums, there is not enough koumiss to organize year-round and uninterrupted koumiss treatment, especially in the winter months. In this regard, in the early sixties, I.A. Saigin with his employees, with our participation, organized a trial drying of mare's milk. The results were positive.

After clinical studies of koumiss obtained from powdered mare's milk at the Shafranovo resort, we came to the following conclusions:

1. Koumiss from dried mare's milk is in no way inferior to natural koumiss in terms of its taste and medicinal qualities.

2. The production of koumiss from dried mare's milk makes it possible to cover the shortage of natural koumiss in the winter months and creates an opportunity for organizing winter and, therefore, year-round koumiss treatment.

3. Drying mare's milk is quite possible on an industrial scale.

The taste, aroma and medicinal properties of koumiss largely depend on the technology of preparation. but koumiss production is still in the hands koumiss makers-practitioners which are getting smaller and smaller every year.

There is no scientifically substantiated technology of koumiss-making in the form of approved technological instructions, instructions and recommendations. There is no corresponding scientific center in the country that would comprehensively solve zootechnical issues of breeding work on breeding and improving more productive and economical breeds of horses and their proper use, technological issues of obtaining high-quality milk, cultivating standard koumiss from it, production, storage and transportation of koumiss and koumiss starter cultures, the biological value and therapeutic possibilities of koumiss of different varieties were studied in the treatment of patients different profile.

So, koumiss occupies a significant place in medical practice. In this regard, it is necessary to increase the production of koumiss everywhere and in every possible way, which is possible with an increase in the number of dairy mares in all republics and regions of the Russian Federation and beyond.

The strengthening of the Lesnoye sanatorium made it possible to organize the production of koumiss and its treatment of tuberculosis patients on a large scale, under the guidance of highly qualified specialists. To obtain koumiss, a starter culture is added to mare's milk, consisting of yeast and lactic acid bacteria, under the influence of which alcoholic and lactic acid fermentation occurs. As a result of fermentation, lactic acid, carbonic acid and alcohol are produced. For sourdough, pure cultures of yeast and lactic acid bacteria are currently used.

The table shows that mare's milk in terms of sugar, protein and dry residue is close to human milk, yielding to it in the amount of fat. After processing mare's milk into koumiss, the amount of sugar in it decreases and lactic and carbonic acid and alcohol appear. Koumiss differs in varieties: 1) weak, 2) medium and 3) strong. Such a division is justified by the fact that the effect on the patient's body of each of the compositions of koumiss is somewhat different.

So, weak koumiss often causes flatulence and intestinal disorders, strong koumiss, on the contrary, causes constipation. In addition, strong koumiss contains the largest amount of alcohol, which is often undesirable. As a rule, medium koumiss is used for medicinal purposes.

The criterion for dividing by varieties was previously considered the age of koumiss: one-day was considered weak, two-day and older - medium and strong. Currently, the variety of koumiss is determined by the degree of acidity. So, in weak koumiss, the acidity is 60-70 ° according to Turner, on average - 80-85 ° and strong -105-115 °. The amount of alcohol usually increases in parallel with the formation of acids, so the degree of acidity fairly reflects the chemical composition of koumiss.

The effect of koumiss on the patient's body depends on its chemical composition. This is a good nutrient. As a carbonated, sour drink, koumiss is well tolerated in large quantities and provides a source of nutrition through its protein and fat content. Proteins of koumiss are well absorbed and digested. Thanks to this, as a result of koumiss treatment, nitrogen balance is restored, and fat is also deposited. The lactic and carbonic acids contained in koumiss, as well as a small amount of alcohol, have an exciting effect on the glands of the digestive tract, which contributes to the digestion, absorption and assimilation of other types of food.

For the same reason, koumiss has a pronounced juice effect, which helps to increase appetite and improve gastric digestion. These properties of koumiss contribute to the increase in the patient's weight, improve his general condition, strengthen the central nervous system, increase its tone and gradually normalize the functional state.

This is facilitated by a slight irritating effect on the nervous system through the receptors of the internal organs of the individual components of koumiss - lactic and carbonic acid and alcohol. The patient improves sleep, mood, reactions to the environment become more favorable. All this contributes to a total increase in the body's resistance, active mobilization of its defenses. Thus, koumiss has a stimulating effect on the body of a tuberculosis patient.

The acids that make up koumiss contribute to the normalization of metabolism, acid-base balance. In addition, carbonic acid has an irritating effect on the respiratory center, thereby contributing to the deepening of breathing and, consequently, better ventilation of the lungs and an increase in oxidative processes. Lactic acid acts depressingly on the vital activity of Escherichia coli, as a result of which autointoxication of the body decreases.

Due to the above properties, koumiss is one of the means that occupy a significant place in the complex of methods for the treatment of tuberculosis, contributing to the subsidence and subsequent cure of the tuberculosis process. As a result of treatment, the phenomena of general intoxication decrease, the temperature decreases; signs of a more favorable course of the tuberculous process in the lungs and its tendency to subside are expressed in the disappearance of catarrhal phenomena, scarring of tuberculous foci.

The success of treatment with koumiss depends to a large extent on the method of treatment. The correct dosage matters, since the indiscriminate use of koumiss in excessive doses and in cases not indicated can have a negative effect on patients. Digestive disorders, shortness of breath, palpitations, excitation of the nervous system, hemoptysis, fever may appear. It is necessary to start koumiss treatment no earlier than 2-3 days after arrival at the sanatorium, so that the body has time to somewhat acclimatize in the new conditions and take a break from the impressions associated with them.

Koumiss should be drunk warm on an empty stomach or after a light breakfast, in small sips and finished 1-1/2 hours before the next meal. On the first day, the amount of koumiss drunk should be no more than 200-500 ml per day. In the following days, this amount increases and is brought up to 1500-3000 ml per day. The total daily dose is set individually, depending on the general condition of the patient, the nature of the disease process, concomitant diseases, the patient's habits and his individual response to small doses of koumiss. The total daily amount is distributed so that in the first half of the day the patient drank 2/3, and in the second - 1/3 of this amount.

During treatment, the patient is assigned a certain regimen, which should provide for the correct alternation of sleep and wakefulness, time for meals, walks, entertainment, and certain hours are set in which the patient should drink one or another amount of koumiss.
